2025 marks a meaningful milestone for CHS Healthcare, 30 years of guiding families across the UK to find the right care for their loved ones, when it matters most.
For more than three decades, we’ve helped thousands of families navigate one of life’s most difficult and emotional decisions: finding the right care for someone they love. From day one, our mission has been more than simply presenting options, it’s been about offering compassionate, expert guidance that empowers people to make confident, informed care choices.
Our story began in 1995, under the name Care Home Selection, with a simple but powerful mission: to support families through the process of finding the right care. We’ve always believed that no one should have to face these decisions alone. While we don’t provide care ourselves, we stand beside families, offering clarity, compassion, and confidence through every stage of the journey.
We understand how overwhelming and emotional these moments can be. That’s why, from the very beginning and still today, we’ve made it our mission to listen closely, explain options clearly, and connect people with trusted, CQC-accredited providers, always with empathy, and at no cost to the families we support.
As our service grew, so did the needs of the families we supported and the professionals we worked alongside. In response, we expanded beyond care-finding to include hospital discharge coordination and care assessments, evolving into what became known as CHS Healthcare, the name we proudly carry today.
Around this time, CHS Healthcare proudly joined Acacium Group, becoming part of a wider family of innovative healthcare organisations. Being under the Acacium Group umbrella enabled us to expand our impact, working more closely with NHS teams and local authorities to help people move safely and confidently from hospital to home or into ongoing care. We’ve been privileged to support families during some of life’s most challenging moments, and to collaborate with dedicated healthcare professionals across the country.
Throughout that journey, care-finding remained at the heart of what we do, a consistent, trusted service that people continue to rely on, when it matters most.
In 2024, CHS Healthcare made a strategic decision to transition its hospital discharge and care assessment services to Xyla, a specialist health, wellbeing and patient flow provider within the Acacium Group. This transition allowed those services to continue growing within a broader clinical infrastructure, fostering innovation and integration across the care system.
For CHS Healthcare, it marked a return to our roots, focusing on our core mission of providing compassionate, expert care-finding support to families.
Today, CHS Healthcare proudly focuses on the service that started it all: care finding. As part of Acacium Group, we continue to help families navigate and select the right care home or home care provider, offering expert, independent guidance tailored to their unique needs.
We partner with a growing national network of CQC-accredited providers to offer expert support at no cost to families. Having helped over 45,000 families, our promise of ‘confidence in every care choice’ reflects our commitment to trusted partnerships, personalised guidance, and timely, compassionate service, when it’s needed most.
